Made in the USA for Montgomery Ward, it is Lexington model 71-3635 in the color 277 "Blue Patch". It has 3 pieces including the back, seat/arms and what appears to be a skirt for the base. This slipcover has been stored since the 60s or 70s and is in need of laundering due to a musty odor (and decades of dust!)
